Population health management is definitely in vogue and is the focus of a lot of energy and effort. As integrated Care Systems develop all local partners will be required to work together and to take responsibility, for system objectives in relation to the use of NHS resources, performance and population health.
This interactive and practical workshop event is being delivered in collaboration with Conor Burke and CPB Healthcare Consulting, and will explore the questions - What does Population Health Management really mean, why is it important and how will it help the delivery of better health and care?
We are seeking to engage with opinion formers across the NHS, local government and the supplier community to contribute their thinking and experience around how population health management will have real impact.
